PHYSICIAN PAYMENT
You must pay the total bill, less
the applicable discount at the time
service is rendered. The discount
applies only to services provided by
the physician and performed in the
physician's office. The Provider
will call Beech Street Network to
determine the exact amount of
discount which will be reflected on
your bill.
HOSPITAL PAYMENT
The facility will fax/mail bill to
network to determine exact amount.
If the procedure amount will be more
than $1,000, you must pay $1,000.00
at the time service is rendered.
You will then receive a billing
statement detailing your savings and
the amount you must pay within 30
days of service. For procedures
less than $1,000, you are required
to pay a portion of the full amount
determined by the hospital at the
time service is rendered. The
balance will be due within 30 days
of you receiving the re-priced
billing statement.
