What is ScriptSave?

ScriptSave is a discount drug card, which is an additional free benefit with the Medical Income plan. The ScriptSave card allows a discount of from 15% to 50% on drug purchases. The size of the discount depends on whether the drug is a generic or name brand drug. Generally, the more expensive the drug, the larger the discount, with the average discount being around 22%.

Your ScriptSave card will be mailed to you separately, after you receive your Medical Income plan information.

What are "pre-existing medical conditions", and are they covered under the Medical Income plan?

A pre-existing medical conditions is an illness or injury for which the Insured person has sought medical treatment within the past 12 months prior to the effective date of coverage. They are covered under the Medical Income plan after the Insured has been on the plan for 12 months.

How are Medical Income claims paid? What do I do?

Medical Income claims are paid directly to the Insured, unless the Insured assigns the claims to the doctor or hospital at check-in time.

If you assign the claim to the hospital (this is usually done at admission time), the hospital will fill out and send in the claim form. If you do not assign the claim to the hospital, you must fill out the 'Insureds' portion of the claim form and attach all appropriate medical bills (itemized), and send the form and bills to Universal Trucking Benefits Association (at the address listed below).

How are Disability claims paid? What do I do?

You must fill out the Disability Claim form, then have your Doctor fill out their portion of the form. Don't worry about the 'Employer's Statement'. You must then send the ORIGINAL copy to Universal Trucking Benefits Association (at the address listed below). No faxes can be accepted.

How do I submit a claim?

You need to fill out a claim form, which you received in the package which was mailed to you (additional forms may be gotten from the Universal Trucking Benefits Association). Make sure your Doctor fills in any required information, as well. You need to attach all pertinent medical bills, receipts, and documentation, and mail them to:

How can I speed up the payment of my claim?

You can insure that your claim is paid as soon as possible by properly filling out the required claim paperwork and submitting it, along with the associated medical bills/receipts/documentation, etc., to the Universal Trucking Benefits Association at the above address. Any missing or incorrect information will only delay your payment, since we need to resolve the problems before the claim is paid.

How can I make changes to my policy, such as adding or removing dependents?

You must contact the Universal Trucking Benefits Association (follow the 'Contact Us' link, above), by phone, e-mail, fax, or regular mail.

I called the insurance company regarding a billing question I had, but they couldn't help me. What's Up?

The carriers who underwrite the insurance policies are not familiar with the billing procedures set up for the Universal Trucking Benefits Association. We handle deductions, billing, and customer support for the employees and individual contractors, and act as a 'middleman' to the carriers.

If you have any questions or problem with your billing, please call the Universal Trucking Benefits Association - DO NOT contact the carriers directly. We will work with them on your behalf to resolve any problems.
